Island residents who face end-of-life challenges. Especially important is
the partnership that VIHA shares with Victoria Hospice in providing community-based
palliative care services. Home & Community Care Nurses are the first contact for people needing palliative care in our community. A phone call to the Health Unit located in your neighbourhood begins the process of contact and registration with Home & Community Care (HCC) and Victoria Hospice. See below for a list of Health Units and phone numbers. Your neighbourhood HCC Team provides a variety of supportive services to help palliative clients and their families remain independent in their own homes for as long as reasonable and desirable.
