Home & Community Care Nurses are the first contact for people needing palliative care in our community. A phone call to the Health Unit located in your neighbourhood begins the process of contact and registration with Home & Community Care (HCC) and Victoria Hospice. Your neighbourhood HCC Team provides a variety of supportive services to help palliative clients and their families remain independent in their own homes for as long as reasonable and desirable.
HCC staff members are organized into multi-disciplinary teams to ensure coordination of services and consistency in care.
The Home and Community Care Teams consist of:
| • | Home Care Nurses who help people manage their own care by providing nursing assessment, education and counselling, consultation and, when required, hands-on interventions for palliative conditions. |
| • | Case Managers support adults requiring palliative care to remain independent through the coordination of care services, including subsidized home support services, admission to short term hospice beds for assessment and treatment, admission to palliative beds in the community, and referral to other agencies and services. |
| • | Nutritionists support palliative clients through assessment, consultation and education to develop and maintain optimal nutritional health. |
| • | Rehabilitation therapists provide physiotherapy and occupational therapy services to promote and maintain independence and safety at home. |
| • | Social Workers provide consultation and advocacy for financial and complex social needs for palliative clients and their families. |
| • | Home Support Agencies provide specially trained community health workers, organized through case management services, to provide in home personal care and respite for client and their families. |
